• Language: German – This vaccination record is written in German, making it ideal for German-speaking Swiss citizens and residents of German-speaking cantons.
• Use: The vaccination record provides space to record all of a person's vaccinations, including both routine and special vaccinations, such as those required for travel. This allows for clear and consistent documentation of a person's vaccination history.
• Validity: The card is valid for life and should be updated after each vaccination. It is recognized nationwide in Switzerland but can also serve as proof of vaccinations when traveling to many other countries.
• Contents: The card contains fields for information regarding the vaccinations administered, including the date, vaccine, lot number, and the signature of the physician or the responsible healthcare professional.
• Purpose: The vaccination record is particularly important for documenting vaccinations against diseases such as measles, mumps, rubella, diphtheria, and tetanus, as well as for travel vaccinations such as yellow fever or hepatitis.
